Steps for Medical Record Copy Issuance

- Medical certificates, confirmation letters, and copies of medical records can only be issued upon identity verification of the patient, in accordance with the Medical Service Act.
- If a family member or proxy visits on behalf of the patient, a letter of attorney and required documents are necessary.

Required Documents by Applicant Type
- According to Article 21 of the Medical Service Act and Article 13-3 of its Enforcement Rule, the following documents must be prepared for issuing copies of medical records:
(Issuance is not possible without the required documents) - Even when requesting only a radiology CD, the same document requirements apply for patient data protection.
- Patient: Valid ID
- Patient’s Family (Next of Kin): Applicant's ID, Consent Form, and documents verifying family relationship (e.g., Family Registry Certificate or Resident Registration Certificate)
- Designated Proxy: Applicant's ID, Patient's ID, Consent Form, Power of Attorney

- Download Confirmation Form
- Consent and power of attorney forms must be in the legally designated formats provided by the Enforcement Rule of the Medical Service Act. Other forms are not legally valid.
